The Effect of Atropine Premedication on the Oculocardiac Reflex


Abstract
We observed the effect of intramuscular atropine on EKG changes which were developed due to the oculocardiac reflex following the compression of eyeball or traction of extraocular muscle during strabismus operation in healthy children and compared it with the non-atropinized control group.
The results were as follows;
1. In the non-pretreated control group, the abnormal electro-cardiographic findings were occured in 42 children among 61 children(68.8% )
2. In atropine(0. 03 mg/kg i. m.) premedication group, the abnormal findings were observed in 5 children among 21 children(23.8% )
3. These electrocardiographic changes in atropine premedication group were appeared as ventricular bigeminy in one case(4.8%), decreased heart rate in two cases(9.5%) and pacemaker shifting in two cases(9.5%)
4. The oculocardiac reflex could not be completely prevented by use of atropine, but fairly good results were obtained.
This means that intramuscular injection of atropine(0.03/ kg) should be given prior to ophthalmic surgery and continuous EKG monitoring is necessary to prevent and manage the oculocardiac relfex.
